A "software problem" in the brain

Functional Neurological Disorder (FND) is a complex medical condition in which there is a problem in the functioning of the central nervous system, without there being a structural lesion or degenerative disease visible on MRI scans. To use a metaphor: the "hardware" of the brain is intact, but the "software" (the way it sends and receives messages) is malfunctioning. The symptoms are real and intense.

Most common manifestations

  • Motor Symptoms
Sudden weakness, paralysis of a limb, tremors, body spasms, abnormal movements or changes in gait.
  • Dissociative Crises
Episodes that resemble epileptic seizures, fainting or blackouts, but without the abnormal electrical activity in the brain typical of epilepsy.
  • Sensitive Symptoms
Deep numbness, persistent tingling, chronic generalised pain or temporary loss of vision/hearing.
  • Cognitive symptoms
Brain fog, extreme fatigue disproportionate to the effort, confusion and difficulty concentrating or speaking.

MAIN MANIFESTATIONS OF PNF

Functional Weakness and Paralysis

Sudden or progressive loss of strength in a limb or side of the body, simulating a stroke, but without damage to nerves or brain tissue.

Engine

Movement Disorders

The appearance of functional tremors, dystonia (abnormal muscle contractions), body spasms or profound changes in the gait pattern.

Movement

Non-Epileptic Seizures (CNEF)

Attacks or convulsions that resemble epilepsy, often accompanied by a strong dissociative component (feeling of "switching off").

Crises

Sensory and Cognitive Symptoms

Areas of the body with loss of sensation, severe chronic fatigue, mental fog and persistent problems with speech, vision or swallowing.

Sensitive / Cognitive

PNF can be triggered by physical stress (injury, surgery, infection) or psychological stress, but in many cases it comes on suddenly without an obvious trigger.

EFFECTIVENESS IN TREATMENT
DA PNF / FND

Clinical results consistent with a neuropsychiatric, motor rehabilitation and psychotherapy approach

59%
reported improvement with specialised physiotherapy for functional motor disorder, versus 38% with usual care
47%
reported global health improvement at 5 months with physiotherapy combined with CBT, versus 16% in the comparison group
1,98×
greater likelihood of becoming functionally seizure-free when CBT is added to standardised treatment in forms with dissociative seizures
86%
likelihood of greater cost-effectiveness for specialised physiotherapy compared to usual treatment

THE IMPORTANCE OF A VALIDATING DIAGNOSIS

For many years, patients with FNP were deeply misunderstood, being told that their disabling symptoms were simply "psychological", or that there was "nothing wrong" because the tests were normal. A specialised neurological and psychiatric diagnosis is crucial to validate the symptoms, reliably exclude other pathologies, and clearly explain the mechanism of the disease, which must be treated.

💡 Understanding and accepting the diagnosis ("the hardware is fine, the problem is with the software") is the first and most important step towards rehabilitation.

At Clínica NeuroPsyque, we offer an integrated assessment and a personalised rehabilitation plan that combines specialities such as neurology, psychiatry, psychology and physiotherapy. In addition, we have advanced neuromodulation technology at our disposal, such as Transcranial Magnetic Stimulation (TMS), which has proved central to the treatment of neurological and neuropsychiatric conditions. Our team is prepared to help "reprogramme" your nervous system, restoring brain communication pathways and giving the patient back control of their body through modern, validated treatments, avoiding common complications from excessive and prolonged use of medication.

Frequently Asked Questions

FAQ's about PNF / FND

How is the diagnosis made if tests such as MRI show nothing abnormal?
The diagnosis of FNP is complex and is not made solely by excluding other pathologies. The neurologist and psychiatrist assess the clinical history and look for specific "positive" signs in the neuropsychiatric physical examination (such as the "Hoover Sign" for leg weakness) that demonstrate typical signs of FNP.
Is PNF curable or is recovery possible?
Yes. The reversal potential of PNF is high precisely because it is a "software" problem (functional) and not a "hardware" problem (irreversible damage). The brain retains its neuroplasticity - the ability to relearn how to send the right signals. With the right treatment, many patients achieve total remission or substantial improvement.
What does treatment for FNP consist of?
Effective treatment involves different integrated specialities. It can include specialised neurological physiotherapy (to re-educate automatic movements), psychotherapy (such as Cognitive Behavioural Therapy) to treat factors such as stress, trauma or anxiety that "out of tune" the nervous system, and neuromodulation methods (such as Transcranial Magnetic Stimulation - TMS) or pharmacological adjustment in certain cases.
How does neuromodulation help in the treatment of PNF?
In Functional Neurological Disorder (FNP), neuromodulation acts on the brain circuits involved in controlling movement, perception and emotional regulation, helping to “reorganise” altered patterns of functioning. Techniques such as Transcranial Magnetic Stimulation (EMT), the Transcranial Direct Current Stimulation (tDCS) and Neurofeedback can greatly reduce symptoms, improve motor control and facilitate functional recovery, especially when integrated into a multidisciplinary therapeutic plan.
Is PNF the same thing as Anxiety or Depression?
No. Functional Neurological Disorder is an isolated diagnosis of its own, affecting motor and sensory pathways. However, it is very common for there to be comorbidities (associated conditions) - a exhaustion, a anxiety, a depression or past traumas often act as precipitating or perpetuating factors that put the nervous system in a state of "alert and block".
What triggers the disease?
It can be a combination of previous vulnerabilities and immediate triggers. Often, the trigger is an actual mild physical trauma (a minor injury, a serious infection, surgery) or acute emotional stress, which acts as the "final straw" and causes the functional "short circuit" in the brain.
Do I need a medical referral to book an appointment at NeuroPsyque?
No, referral is not necessary. If you suffer from the symptoms mentioned, or if you have already been diagnosed with PNF/FND and are looking for specialised care, you can book your diagnostic assessment (Neurology) directly with us. Contact us.
