Realising which of the two you have is the first step towards knowing what to do and treating the pain properly.<\/p><p>There is a <strong>single track<\/strong> to separate them, and one <strong>third scenario<\/strong> which many people aren\u2019t aware of. We\u2019ll get to that in a moment.<\/p><h2>What Is Lower Back Pain (Lumbago)?<\/h2><p>A <strong>lower back pain<\/strong>, or <strong>low back pain<\/strong>, is pain felt in the lower back, between the lower ribs and the buttocks. It almost always originates in the muscles, joints or discs of the spine. You can find a general overview on our page about the <a href=\"https:\/\/neuropsiquiatria.pt\/en\/dor-lombar-e-cervicalgia\/\"> <span style=\"text-decoration: underline;\"><strong>treatment of lower back pain and neck pain<\/strong><\/span><\/a>.<\/p><h3>Localised and Mechanical Pain<\/h3><p>This pain is described as <strong>mechanics<\/strong> because it varies with movement and posture: it gets worse with certain activities or after spending a long time in the same position, and eases when you change position.<\/p><p><strong>It usually has these characteristics<\/strong>:<\/p><ul><li>remains <strong>concentrated on the back<\/strong>, in the lower back, and may extend to the buttocks;<\/li><li>feels like a heaviness, a twinge or tension;<\/li><li>It does not usually cause a tingling sensation or weakness in the leg.<\/li><\/ul><h2>What is Sciatica?<\/h2><p>A <strong>sciatica<\/strong> It is not a disease in itself: it is the <strong>symptom<\/strong> caused by an irritated or pinched nerve in the lower back. The medical term is <strong>radiculopathy<\/strong> \u2014 \u201croot\u201d because the problem lies at the point where the nerve exits the spine.<\/p><h3>When Pain Runs Down Your Leg<\/h3><p>It\u2019s the hallmark of sciatica: the pain <strong>radiates<\/strong>, that is, it \u201ctravels\u201d from the lower back or buttocks down the thigh and, often, <strong>reach past the knee<\/strong>, spreading to the leg and foot. It is often described as an electric shock or a burn, and may be accompanied by:<\/p><ul><li><strong>anthill<\/strong>;<\/li><li><strong>dormancy<\/strong>;<\/li><li><strong>weakness<\/strong> on the foot or leg.<\/li><\/ul><h3>The Role of the Sciatic Nerve<\/h3><p>O <strong>sciatic nerve<\/strong> It is the largest nerve in the body.<\/p><p>It originates from the nerve roots in the lumbar and sacral regions \u2014 the lower part of the spine, just above the coccyx and between the buttocks \u2014, passes through the buttock and runs down the leg to the foot. This simple map helps to distinguish between the two conditions \u2014 and provides valuable information for the consultation.<\/div><h2>Lumbosciatica: When the Two Types of Pain Coincide<\/h2><p>This is the <strong>third scenario<\/strong>. Often, both types of pain occur <strong>at the same time<\/strong>: back pain and pain radiating down the leg. It is called <strong>lumbar sciatica<\/strong>.<\/p><p>It is not a \u201cdisease\u201d in its own right, but the combination of the two conditions. In such cases, what matters most is to realise <em>how far<\/em> The pain sets in, and there may be neurological symptoms, such as a tingling sensation or weakness.<\/p><h2>What Usually Causes Sciatica<\/h2><p>According to science, the two main causes are <strong>herniated disc<\/strong> \u2014 the disc, the \u201ccushion\u201d between two vertebrae, slips out of place slightly and presses on the nerve root \u2014 and the <strong>spinal canal stenosis<\/strong>, due to spinal degeneration.<\/p><p>The good news is that most cases are <strong>self-limiting<\/strong>, resolving within about six weeks. The most conservative treatment, which includes the <a href=\"https:\/\/neuropsiquiatria.pt\/en\/fisioterapia\/\"><span style=\"text-decoration: underline;\"><strong>physiotherapy<\/strong><\/span><\/a>, this is usually the first approach.<\/p><h2>Warning Signs That Require Prompt Assessment<\/h2><p>Most cases of back pain and sciatica are not serious. But there are signs that warrant <strong>urgent assessment<\/strong>.<\/p><div class=\"nota-importante\">If you have <strong>loss of control over urine or stools<\/strong>, <strong>numbness in the genital area or between the legs<\/strong> or <strong>weakness in one leg that is rapidly getting worse<\/strong>, go straight to A&amp;E without delay.
